We’ve made it easy for you to fill out patient in-take forms, so you can focus on what really matters the most: your mental health. Read below for a quick step-by- step process on how to get registered with us.

Browse our specialists to find a provider who may best fits your needs, or discuss with us your goals in selecting a provider

Step 2

Fill out Paperwork and submit it. Our doctors do not take insurance, but some clinicians offer a sliding scale.

Step 3

After paperwork submission, you will be contacted via email to schedule an appointment.

Step 4

You and your clinician will develop a personalized treatment plan and begin working together to reach your wellness goals.

Frequently asked questions

We do not take insurance, but we do provide receipts that you can submit to your insurance if you have out-of-network benefits.
Yes, we have clinicians that offer lower fees for patients who do not have out-of-network benefits and meet income requirements. Please send us an email or give us a call to start your application.
It depends on which doctor you would like to see. We have many clinicians who can start treatment within a week of you submitting paperwork.
You may select a clinician or discuss with us to determine the best fit. After that, you can submit the new patient paperwork, and then we can begin scheduling you.
Yes, there typically is a waiting list to see Dr. Hollander. He has hand selected the top clinicians who you can meet with in place of him or while you wait to schedule an appointment with him.
We accept all major credit cards, debit cards, and Zelle.
Of course, we are collaborative practice and enjoy working with outside clinicians. There can be benefits to seeing a psychologist and psychiatrist in a group practice. It is more important to us that our patients are receiving the proper care and that often includes patients seeing clinicians outside of our practice.
